UAW Local 72 Retirees and Surviving Spouses
Humana Medicare Advantage Plan Network Update

We would like to caution retirees and surviving spouses in the Humana Medicare Advantage Plan to avoid utilizing out-of-network health care provider services, if at all possible. Utilizing out-of-network providers will cause you to spent large amounts of money out of your own pocket in any of the health care plans, including the Humana Medicare Advantage Plan.

In areas with a limited number of network-participating providers, to avoid any unnecessary out-of-pocket expense, call the Humana Customer Care (customer service) phone number on the back of your ID card before treating at an out-of-network facility and request to be allowed to see an out-of-network provider or to go to an out-of-network facility.

Health Care Premium Vouchers for Retirees and Surviving Spouses Who Do Not Receive Pensions

UAW Retiree Medical Benefits Trust (UAWRMBT), more commonly known as the VEBA, will be sending a Direct Billing Notice to all non-pension eligible retirees and surviving spouses on May 10, 2010. The invoice is for six months of premiums for VEBA-provided health care and prescription drug coverage for January through June 2010.  Retirees and surviving spouses who receive these invoices (bills) must pay the full amount billed before June 1, 2010 to avoid cancellation of VEBA-provided health care and prescription drug coverage. Health care premiums are $15 per month for one covered member, and $30 per month for two or more covered members in the same household. Households with single coverage will receive a bill for $90 for January through June of 2010. Households with more than one covered member will receive a bill for $180 for January through June of 2010.

Humana Medicare Advantage Plan and Prior Authorization

There are several medical procedures that require prior authorization (PA). Your health care provider (not you) is responsible for requesting a PA from Humana. We are finding that a number of these PA’s are being denied by Humana. If Humana denies your provider’s request for prior authorization, please contact the Union Benefit Office at the numbers below.

Medicare Part D Cancellations

As we wrote last month, Medicare has cancelled Medicare Part D prescription coverage for retirees enrolled in the Humana Medicare Advantage Plan. If your Medicare Part D coverage was terminated, and you wish to re-enroll in it, you must switch from the Humana Medicare Advantage Plan to the Humana Traditional Care Network Plan by writing to Humana at the following address:
HUMANA
PO Box 14168
LEXINGTON, KY 40512-4168
Or, you may fax your letter to Humana at 1-800-633-8188. Your signature must appear at the bottom of the letter.

Did You Opt Out of the Humana Medicare Advantage Plan?

If you wrote or faxed Humana to opt out of the Humana Medicare Advantage Plan and  to enroll in the Humana Traditional Care Network Plan, and received a letter from  Humana stating you have no health care coverage, please do not panic.  Even though it is not mentioned in the letter, the intent is to inform you that after your Humana Medicare Advantage Plan has is cancelled, you will be enrolled in the Humana Traditional Care Network Plan and will continue to have prescription drug coverage with MEDCO as your drug carrier.  If you opted out of the Humana Advantage Plan, please contact the Union Benefit Office at one of the phone numbers that appear at the end of this notice. We are finding that Humana is not enrolling some of the retirees in the Traditional Care Network Plan in a timely manner without an additional call from the retirees or union benefit reps. So, please also call the Humana Member/Provider phone number on the back of your ID card to make sure their enrollment department enrolls you in the Humana traditional Care Network Plan as requested.

Illinois Retirees Formerly Enrolled in Humana HMO

If you are a non-Medicare eligible UAW-Chrysler retiree living in Illinois, who until the end of  2009 was enrolled in the Humana HMO Plan, please contact the Union Benefit Office at the numbers listed below. We believe you may have been taken out of the HMO Plan and enrolled in the Wisconsin Traditional Care Network Plan of Humana without your consent.
